NEW PATIENT REGISTRATION
We accept new patient registrations from any person wishing to register with us as long as they live within a 4 mile radius of the surgery.
Please ensure that you have downloaded and completed all of the following paperwork (these can be collected in reception) and brought with you to the Surgery all necessary identification documents:
- New patient registration form New Patient Summary
- HS200 (if your previous doctor is in the UK) HS200
- HSCR1 (if you come from outside of the UK or have been de-registered from your previous practice for over a year) HSCR1
Please provide proof of address by way of a recent utility bill, or other documents and photographic identification.
If you are transferring from another GP Surgery, you must submit a List of medications from your current GP with the registration forms.
The Practice does not discriminate on the grounds of race, gender, social class, religion, sexual orientation, appearance, disability or medical condition.
Patients who are violent or abusive towards GPs or staff members may be removed from the Practice list. Patients who threaten or are violent towards any GP or staff member will be reported to the Police and further action pursued. Zero tolerance policy
The practice accepts completed registration forms on Monday, Tuesday, Thursday and Friday afternoon from 2pm to 6pm.
